MENTORING AGREEMENT

Before requesting mentoring read the following agreement between mentor and mentee.

  1. The goal of the mentoring sessions is to help you (the mentee) go through the Recovery Tracker Workbook.

  2. A mentoring session usually lasts about 20 minutes. 

  3. Typically, three or fewer sessions are required to go through the workbook.

  4. Mentoring is free; however, depending on demand, it may not always be available.

  5. Both mentee and mentor are required to relate respectfully to each other during all interactions.

  6. If at any point you have concerns or questions about the mentoring process, please discuss them with your mentor.

  7. Mentoring should only take place so long as both mentee and mentor are completely happy to proceed; in order to facilitate this, each party has the right to terminate mentoring without giving any reason or prior notice.

  8. Any information or guidance shared by the mentor is strictly to be seen as an expression of their opinion and is not in any way to be considered professional advice.

  9. You can choose to share as much, or as little, information about yourself as you feel comfortable with during the sessions. It is perfectly fine not to share any information about yourself as the focus of the mentoring is educational, i.e. to share information about how to use self-tracking tools for recovery and wellbeing.

  10. The information shared in the mentoring relationship, i.e. information shared during mentoring sessions, is confidential to the relationship with the exception of the presence of a significant risk of harm to any person, or neglect of any vulnerable person, being disclosed. If you have any questions about confidentiality please discuss them with the mentor before disclosing any sensitive information so you are fully aware of the likely consequences of disclosing this information. If the information is of a sensitive nature it may be more appropriate and helpful to share it with relevant professionals, as they would have the training and resources to best support you.

  11. Mentoring is not professional support. You should not rely on mentoring as a substitute for, nor does it replace, professional advice, diagnosis, or treatment. If you have a mental health, addiction or physical health concern, you should consult with the relevant professionals for support and guidance.

  12. No assurance of any kind is given that participation in mentoring will improve a person's wellbeing or help them recover from a mental health or addiction concern.

  13. No guarantee is given that the information shared during mentoring is correct, complete or up-to-date. 

  14. Any reliance you (the mentee) place on the information, resources, advise or guidance shared by the mentor is strictly at your own risk. 

  15. By participating in mentoring you agree that in no event will you begin legal proceedings or otherwise attempt to hold the mentor liable for any harm, damages or costs as a result of your participation in mentoring. 

  16. You are expected to read the disclaimer on the website - www.WellbeingTrackers.com - before using any of the tools or resources that can be downloaded from it. 

  17. The mentee is wholly and solely responsible for what he/she chooses to do with the information, guidance and resources provided during mentoring. 

  18. The mentor is not responsible for, and expressly disclaims all liability for, damages or harm of any kind arising out of use, reference to, or reliance on any information, advice or resources shared by the mentor.

  19. Mentoring is only available to individuals that are 18 years of age or older. By requesting and/or participating in mentoring you are identifying yourself as a person that is 18 years of age or older.

  20. This agreement is stated in addition to your legal rights.

  21. By requesting and/or participating in mentoring you are asserting that you have read this agreement and will comply with it.

Steps to request mentoring sessions:

Step 1: Join the "Self-Tracking Recovery" Facebook Group.

Step 2: Read the mentoring agreement.
 

Step 3: If you are happy with the mentoring agreement, send me an email and let me know you are interested in mentoring. Then, depending on availability, we'll schedule some 1-1 mentoring sessions on Zoom to go through the Recovery Tracker Workbook.
